Direct Deposit Forms
Thank
you for your interest in setting up a First Service direct
deposit.
First
Service FCU has two different direct deposit forms. If you
work for the U. S. Government, click on the U. S. Government
Direct Deposit Link.
If
you work for a private employer, you can use the Personal
Direct Deposit Form. Please verify with your employer whether
they have a form for direct deposit first before using ours.
Just
click on the one that applies, print, and fill it out and
return it to the credit union. Our location info is located
under the location tab above. Please enter your 10-digit account
number on the U. S. Government Direct Deposit form from your
check. On the Personal Direct Deposit form it is already annotated
with the right numbers. Just enter your personal credit union
account number.
